To the best of my knowledge, the archdiocese does not object to the use of these materials for your own ministry as a priest, deacon, nullity case sponsor, or someone involved in marriage preparation. The forms that appear on this site are authentic copies that were made either from PDF versions distributed by the archdiocese OR scanned copies of paper forms supplied by the archdiocese. The forms were not modified in any way other than adding the capability of typing information onto the form.
The instructions on the archdiocesan forms usually say, "Please print or type..." These forms provide ministers with something they can "type on with their computer."
Misuse of these forms is possible, and any such misuse is the responsibility of the person involved.
